Community Care provides office and assessment space to the GAIN Clinic for their appointments on Mondays each week in Peterborough.
“It’s a good fit with Community Care and allows us to see patients outside of a hospital setting,” said Stacey Moore, RN(EC)-Adult, Nurse Practitioner – GAIN. “It gives us a central spot to meet people in the community especially now with COVID restrictions for pre-booked appointments.”
What is the GAIN Clinic?
The Geriatric Assessment and Intervention Network (GAIN) Clinic, under the umbrella of Peterborough Regional Health Centre, is part of a network of coordinated healthcare services and provides clinical care and education to ensure the best care for frail older adults and their families.
Services include:
- Comprehensive geriatric assessment
- Assessment of memory and thinking
- Assessment of balance, mobility and falls risk
- Medication and supplement review
- Management of responsive behaviours
- Caregiver support and navigation
The GAIN clinic also offers interventions to patients, including:
- Education regarding chronic disease self-management
- Teaching about dementia and behavioural care
- Advance care planning
- Referral to community supports
- Where appropriate, collaboration with home care to facilitate in-home supports, long-term care application and more
- De-prescribing and medication adjustment
- Initiation of treatment for dementia
This inter-professional team of healthcare professionals works collaboratively with patients and families and their primary care providers to strengthen supports and capacity for older adults whose health concerns threaten their ability to live independently at home (including retirement residences).

- Personal Support Workers
- Physiotherapists
- Pharmacists
- Social Workers
The GAIN Clinic accepts referrals from any source, including nurse practitioners, physicians, and patients and families directly.
