Eligibility for services is based upon client service criteria, assessed need and priority setting. Services that are provided are intended to help the people receiving support maintain their independence in the community.
Once we have received your information, a staff member from Community Care will contact you to discuss our services with you, either over the phone or in person. They will help you understand how the services will be delivered to you, and go over any fees that may apply.
Some services may not be provided in all offices.
