What is HAL?
Home At Last is a program funded by the Central East Local Health Integration Network (LHIN) to help clients successfully make the transition from hospital to home. HAL is a partnership between Peterborough Regional Health Centre (PRHC), the Central East Community Care Access Centre (CE-CCAC) and Community Care Peterborough (CCP).
Who is HAL meant to serve?
HAL services are available without cost to adults living in Peterborough City and County who are aging, frail, or have special needs and may benefit from additional support settling in at home upon discharge from PRHC. For those who live outside of Peterborough City and County, HAL Peterborough may be able to connect clients with the service in their region.
Who can make a referral to HAL?
Referrals may be made by any member of the client’s care team at PRHC , e.g. social workers, case managers, charge nurses, therapists, physicians, discharge planners. Individuals and family members may also refer to HAL.
When are HAL services available?
HAL services can be used Monday through Friday, excluding statutory holidays. Referrals and inquiries can be made weekdays between 8:30 a.m. and 4:00 p.m. by calling 872-6850.
What are “settling in” services?
“Settling in” tasks help clients resume their lives at home following a hospital stay. They are performed by a Personal Support Worker employed by Community Care Peterborough and include:
- Accompanying/driving a client home and making sure she/he is safe and comfortable.
- Picking up medications and/or groceries
- Making a meal
- Doing light housekeeping
- Conducting a safety assessment
- Providing personal care
What kind of follow-up is done?
The HAL Coordinator calls or visits the client during the first week to learn how they are settling in and determine if any additional community supports are needed.
