Request for Services
You can contact any Community Care office to register for our services. Many people are referred to us by their physician, another health professional, family member or friend who is familiar with us. Registration is required for most services, except seminars and workshops.
Eligibility for services is based upon Client Service Criteria, assessed need and priority setting. Services that are provided are intended to help the people receiving support maintain their independence in the community.
If you are interested in any of our services for yourself or a family member, please complete the attached Service Application and fax/mail to the appropriate local office.
Once we have received your information, a staff member from Community Care will contact you to discuss our services with you, either over the phone or in person. They will help you understand how the services will be delivered to you, and go over any fees that may apply.
